For making declarations of will and signing on behalf of the company, the President of the Management Board, a member of the management board or a commercial proxy of the company is authorised independently, subject to the restrictions arising from points a and b; a) the President of the Management Board acting jointly with a member of the management board or a commercial proxy of the company in the case of concluding transactions disposing of assets or creating an obligation with a value exceeding PLN 20,000.00 (twenty thousand złoty 00/100); b) the President of the Management Board acting jointly with a member of the management board or a commercial proxy of the company in the case of performing legal acts consisting in: concluding, withdrawing from or terminating an agreement with the National Health Fund for the provision of primary health care; concluding, withdrawing from or terminating an agreement with the company's subcontractors for the provision of medical services falling within the scope of an agreement concluded by the company with the National Health Fund or covered by financing from public funds.
